Enrollment Information and Forms

In order to complete the registration process you will need to bring the following documents:

1. Proof of Residence (must be dated within 30 days of enrollment):
   1. Utility Bill in parent/guardian’s legal name (Gas, Water, Electric only.)
   2. Note that phone bills, driver’s licenses as proof of address, or other mail are NOT accepted
2. Parent/Guardian Identification (photo ID)
   1. State Identification (Valid Identification Card, Driver’s License, Consulate I.D.)
   2. Court Order establishing parent/guardian/educational rights
3. Student Proof of Age and Identity
   
   1. Original Birth Certificate, Baptismal Certificate, Passport
4. Immunization Record/Card
5. Report Card from the last school attended / Most Recent Transcript
6. IEP or 504 Plan (if applicable)
7. Current Court Orders (if applicable)
